Please arrive about fifteen minutes before the time of your appointment to check in. We will have copies of your medical file. However, if you have imaging studies that are less than two months old you should arrange to pick those up and bring them with you. You may bring a friend or family member to any examination except one with a psychiatrist, but children are not allowed in the exam rooms.

You will be asked to fill out a form with information about your injury and past treatment. If you want to fill out the history portion ahead of time, click the "Info Sheet" button on this page. You will also be asked to sign the HIPAA form, which can be seen by clicking the "HIPAA Form" button.

You will then meet with one or more physicians to go over your medical records. Depending on the size of your file, this could take 20-60 minutes. You will then have the physical portion of your evaluation, which will include doing various movements and having measurements taken. It will not include any injections or needles! The exam itself will only take 10-15 minutes. If the physicians feel that tests or imaging are needed, our office staff will set up a time with you for those. Psychiatric evaluations do not include a physical and take about and hour and a half.

The physicians cannot discuss their findings with you. They will send a report to your claims manager or attorney about seven days after the evaluation. We are not allowed to send you a copy of your report.
